hypothalamic-pituitary-gonadal axis and are associated with either excess
prolactin
or decreased testosterone levels. Other
endocrine disorders that may cause libido or erectile difficulties include
hypothyroidism, hyperthyroidism, adrenal insufficiency, or excessive levels
of adrenal corticosteroids." - see

The relationship between
serum 25-hydroxy vitamin D and arteriogenic erectile dysfunction -
Andrologia 2022 Aug 29 - "International Index of
Erectile Function-5 (IIEF-5) questionnaire ... peak systolic flow velocity (PSV)
... 25(OH)D levels were significantly lower in patients with organic ED (18.24 ±
6.04 ng/ml) than in patients with psychogenic ED (20.90 ± 8.79 ng/ml) (p =
0.032). In A-ED and NA-ED, the mean of peak systolic flow velocity (PSV) values
was 18.94 ± 5.28 cm/s and 51.57 ± 15.42 cm/s (p < 0.001), and the mean of
25(OH)D was 15.66 ± 5.86 ng/ml and 20.48 ± 5.90 ng/ml, respectively (p < 0.001).
The results showed that 25(OH)D levels were positively correlated with IIEF-5
scores and the PSV values in A-ED patients. The 25(OH)D cut-off value
differentiating between A-ED and NA-ED was 15.05 ng/ml. Low 25(OH)D levels may
be an independent risk factor for ED, especially A-ED. ED patients should
routinely undergo serum 25(OH)D level measurement, and 25(OH)D replacement
therapy is necessary for patients with low vitamin D levels" - See

Early outcomes of
short-course low intensity shockwave therapy (LiSWT) for erectile dysfunction: A
prospective, randomized, double-blinded, sham-controlled study in Malaysia -
Andrologia 2022 Jul 1 - "The International Index of
Erectile Function-5 (IIEF-5) and Erection Hardness Score (EHS) questionnaires
were used for evaluation. Patients underwent weekly sessions for 4 weeks and
were re-assessed at 1, 3 and 6 months post therapy. Fifty one patients were
recruited and randomized into sham and treatment arms. The mean IIEF-5 scores
were significantly improved in the treatment arm compared to worsening of scores
in the sham arm after 1 month (14.1 vs. 9.3 p < 0.001), 3 months (14.9 vs. 8.6,
p < 0.001) and 6 months (14.2 vs. 7.9, p < 0.001) post treatment. A significant
improvement of EHS was demonstrated at 1 month (2.4 vs. 1.8, p = 0.001, 3 months
2.7 vs. 1.7, p < 0.001) and 6 months (2.7 vs. 1.6, p < 0.001) in the treatment
arm compared to sham arm. The success rate based on IIEF score increment more
than five points was 26% in treatment arm and 0% in sham arm. Improvement in EHS
score ≥3 in the treatment versus sham arm was 63% and 4%, respectively. There
was no adverse effect reported. This 4-week LiSWT protocol reflects better
treatment compliance, and it prevents further deterioration of erectile function
among this cohort of patients. This study proves that LiSWT is a well-tolerated
treatment with modest improvements in erectile function and hardness, among
patients with vasculogenic ED"

Trigonella foenum-graecum L.
and Psoralea corylifolia L. Improve Erectile Dysfunction in Streptozotocin-Induced
Diabetic Rats through Suppression of Oxidative Stress - Evid Based
Complement Alternat Med 2022 Jun 6 - "Diabetes
mellitus-induced erectile dysfunction (DMED) is one of the most common
complications of diabetes and is mainly attributed to oxidative stress. Hu-Lu-Ba-Wan
(HLBW) is a classic Chinese formulation consisting of Trigonella foenum-graecum
L. (TFG) and Psoralea corylifolia L. (PC). HLBW has been used not only for the
treatment of diabetes but also for the treatment of erectile dysfunction in
clinics ... The results showed that HLBW significantly reduced blood glucose and
increased insulin sensitivity. HLBW reduced oxidative stress and apoptosis. In
addition, we observed that the expression levels of vWF, α-SMA, and eNOS as well
as the ratio of smooth muscle to collagen increased in the HLBW group"

The relationship between the
gut microbiota, benign prostatic hyperplasia, and erectile dysfunction - Int
J Impot Res 2022 Apr 13 - "Microbiota is defined as the
group of commensal microorganisms that inhabit a specific human body site. The
composition of each individual's gastrointestinal microbiota is influenced by
several factors such as age, diet, lifestyle, and drug intake, but an increasing
number of studies have shown that the differences between a healthy microbiota
and a dysbiotic one can be related to different diseases such as benign
prostatic hyperplasia (BPH) and erectile dysfunction (ED) ... Gut microbiota
modifications can influence prostate health indirectly by the activation of the
immune system and the production of proinflammatory cytokines such as IL-17,
IL-23, TNF-alpha, and IFN-gamma, which are able to promote an inflammatory
state. Gut dysbiosis may lead to the onset of ED by the alteration of hormone
levels and metabolic profiles, the modulation of stress/anxiety-mediated sexual
dysfunction, the development of altered metabolic conditions such as obesity and
diabetes mellitus, and the development of hypertension. In conclusion, much
evidence suggests that the intestinal microbiota has an influence on various
pathologies including BPH and ED" - See

Quercetin
boosts nitric oxide levels and modulates the activities of
arginase, acetylcholinesterase and adenosine deaminase in the
corpus cavernosum of cyclosporine-treated rats - Andrologia
2022 Feb 25 - "One of the primary causes of erectile dysfunction
(ED) in males is cardiovascular disease, such as hypertension
(HT). As a result, the goal of this study is to see how
quercetin (Q) affects the important biochemical parameters
(nitric oxide, endogenous antioxidant enzymes)/specific enzymes
(arginase, acetylcholinesterase and adenosine deaminase) linked
to be responsible for smooth muscle relaxation in respect to
sexual function ... Compared to the control group, Q of both
concentrations reduced the activity of these enzymes and
improved antioxidant status and NO levels" - See

Hyperuricaemia is an
important risk factor of the erectile dysfunction: A systematic review and
meta-analysis - Andrologia 2022 Feb 7 - "Serum uric
acid can affect endothelial function, and hyperuricaemia-induced endothelial
dysfunction is involved in the pathogenesis of cardiovascular diseases. As
endothelial dysfunction is also a main pathogenic mechanism of erectile
dysfunction (ED), the present study aims to evaluate the relationship between
hyperuricaemia and ED via systemic review and meta-analysis ... In the patients
with hyperuricaemia, the risk of ED was 1.59-fold higher than (pooled OR = 1.59,
95% CI [1.29, 1.97]) the non-hyperuricaemia counterparts. Urate-lowing therapy
(ULT) in these hyperuricaemia patients reduced the risk of ED by 27% (OR = 1.27,
95% CI [1.14, 1.41]). After subgroup analysis, the association between
hyperuricaemia and ED remained significant apart from the >60 years subgroup.
Hyperuricaemia is an important risk factor of ED, while ULT can reduce the risk
of ED in hyperuricaemia. This study suggests that hyperuricaemia-associated
endothelial dysfunction may also underlie the pathogenesis of ED in these
patients"

Association between folic
acid, homocysteine, vitamin B12 and erectile dysfunction-A cross-sectional study
- Andrologia 2021 Sep 9 - "FA decreased with the
severity of ED (7.52 versus 6.15 versus 5.49 versus 3.97; p < .001), while HCY
increased (10.35 versus 11.8 versus 12.9 versus 15; p < .001). Smoking and shift
work were associated with lower FA levels. Multivariate analysis showed that
serum FA and HCY revealed significant relation with ED. ROC analysis showed that
FA ≤ 8.84 and HCY ≥ 10.35 were the best cut-off values for ED diagnosis. Both FA
(r = -0.703, p < .001) and B12 (r = -0.576, p < .001) were negatively correlated
with HCY. In conclusion, low FA levels and high HCY levels might be independent
risk factors for ED. Low serum FA and B12 levels might co-cause high HCY levels
and lead to ED" - See Jarrow

Exercise associated or not to the intake of Eurycoma longifolia improves
strength and cardiorespiratory fitness in men with androgen deficiency -
Comp Ther Clin Prac, Feb 21 - "This study aims to
analyze the effects of a concurrent training (CT) associated with Eurycoma
longifolia (EL) supplementation on the muscle strength, cardiorespiratory
fitness, and symptomatology score of men with the androgen deficiency of aging
male (ADAM). Forty-five subjects (47.6 ± 5.2 years) were included in the study.
The randomized, double-blinded, placebo controlled clinical trial lasted for 6
months. Participants were randomized into four groups, control (C; n = 12); EL
(n = 11); CT (n = 11); and CT + EL (n = 11). Along this period, the isokinetic
peak torque of the knee extensors increased in the CT (14%) and CT + EL (17%)
groups (p = 0.040; p = 0.006, respectively), while the isokinetic peak torque of
knee flexion increased in the CT + EL group only (p < 0.05). For all
participants, testosterone levels were correlated with isokinetic peak torque of
knee extension (r = 0.517, p = 0.001) and flexion (r = 0.362, p = 0.028).
Subjects of the CT (27.3%) and CT + EL (36.1%) groups decreased the
symptomatology of ADAM (p = 0.005). This study demonstrated the benefits of CT
and EL consumption as a non-pharmacological treatment for ADAM" - [Nutra

A
6-month, double-blind, placebo-controlled, randomized trial to evaluate the
effect of Eurycoma longifolia (Tongkat Ali) and concurrent training on erectile
function and testosterone levels in androgen deficiency of aging males (ADAM)
- Maturitas 2021 Mar - "Androgen deficiency of aging
males (ADAM) largely manifests as sexual symptoms. Erectile dysfunction is one
of the most common symptoms of ADAM ... 6-month, randomized, double-blind,
placebo-controlled four-arm clinical. 45 men (47.38 ± 5.03 years) were
randomized into 4 groups (G1: control + placebo; G2: control + Eurycoma
longifolia; G3: concurrent training + placebo; G4: concurrent training +
Eurycoma longifolia). 22 received a 200 mg supplement of Eurycoma longifolia and
23 underwent the intervention with concurrent training, 3 times a week for 60
min at progressive intensity ... Erectile function demonstrated improvements in
both interventions; however, the most significant results were obtained by men
allocated to concurrent training + Eurycoma longifolia" - [Nutra

Potential benefits of Berry
Anthocyanins on Vascular function - Mol Nutr Food Res 2021 Aug 3 -
"Cardiovascular disease (CVD), such as hypertension and
atherosclerosis, are the leading cause of global death. Endothelial dysfunction
(ED) is a strong predictor for most CVD making it a therapeutic target for both
drug and nutrition interventions. It has been previously shown that polyphenols
from wine and grape extracts possess vasodilator activities, due to the
increased expression and phosphorylation of the endothelial nitric oxide
synthase (eNOS), and consequent vasodilator nitric oxide (NO) production. This
is vital in the prevention of endothelial dysfunction, as NO production
contributes to the maintenance of endothelial homeostasis. Moreover, polyphenols
have the ability to inhibit reactive oxygen species (ROS) which can cause
oxidative stress, as well as suppress the upregulation of inflammatory markers
within the endothelium. However, while the majority of the research has focused
on red wine, this has overshadowed the potential of other nutritional components
for targeting ED, such as the use of berries. Berries are high in anthocyanin
flavonoids a subtype of polyphenols with studies suggesting improved vascular
function as a result of inducing NO production and reducing oxidative stress and
inflammation" - See blueberry extract at Amazon.com and

Protective effects of wheat
germ oil against erectile and endothelial dysfunction in streptozotocin-induced
diabetic rats - Int J Impot Res 2021 Jun 9 - "Our
aim was to investigate the protective effect of wheat germ oil (WGO) at
different doses on diabetes mellitus (DM)-induced erectile and endothelial
dysfunction ... Acetylcholine-mediated relaxation in the aorta and erectile
functions decreased significantly in the DM group (p = 0.018 and p = 0.005). WGO
(3 and 6 ml/kg) improved vascular functions in the DM groups (p = 0.001 and p =
0.014). The beneficial effect of WGO on erectile function appeared at higher
doses of WGO. However, a higher dose of WGO substantially increased the
oxidative stress parameters in both aorta and penile tissue. These findings
suggest that the improvement in vascular or erectile function by WGO was not
related to antioxidant effects, and new studies are needed to clarify the
mechanism" - See wheat germ oil at

Berberine modulates crucial
erectogenic biomolecules and alters histological architecture in penile tissues
of diabetic rats - Andrologia 2021 Apr 30 -
"Berberine is an isoquinoline alkaloid, found in several plants. Diabetes
induces erectile dysfunction (ED) via reduction in some hormones and enzymes
implicated in sexual function. This study aimed to investigate the role of
berberine on crucial biomolecules linked to penile function in diabetic rats.
Sixty-three (63) adult male rats were used and distributed into nine groups
(each = 7). Group I-IV normal rats administered with citrate buffer (pH 4.5),
sildenafil citrate (SD, 5.0 mg/kg), 50 and 100 mg/kg of berberine, respectively,
via oral gavage. Rats in groups V-IX were diabetic rat with ED treated with
buffer, SD, 50 and 100 mg/kg of berberine, and acarbose (25 mg/kg ACA)
respectively. The result revealed that histological architecture in penile
tissues were altered in diabetic groups treated with berberine, sildenafil
citrate and acarbose when compared to the diabetic control group. Treatment with
berberine, increased testosterone, luteinizing hormone and follicle-stimulating
hormone in diabetic rat with ED. Also, reduced prolactin level and
acetylcholinesterase, angiotensin-1 converting enzyme, adenosine deaminase and
arginase activities were observed in berberine treated diabetic rat with ED.
Molecular docking analysis revealed that berberine had strong binding affinities
for these enzymes. Thus, berberine could represent a potential therapeutic agent
for diabetes-induced ED." - See berberine at Amazon.com.

Restorative effects of red
onion (Allium cepa L.) juice on erectile function after-treatment with 5α-reductase
inhibitor in rats - Int J Impot Res 2021 Apr 5 -
"Benign prostatic hyperplasia (BPH) is one of the most prevalent conditions
among aged men. The use of 5α-reductase inhibitors (5-ARIs) to treat BPH was
linked to erectile dysfunction (ED). Many medicinal plants and secondary
metabolites are used in the management of ED. Onion (Allium cepa L.) is an
economically affordable vegetable with vital phytochemicals and biological
functions. The study aimed to identify the beneficial effects of onion juice on
dutasteride (a 5-ARI)-induced ED. Rats were divided into two groups (n = 5 per
group): control and dutasteride-treated rats (0.5 mg/kg/day). Dutasteride was
administered in drinking water for 12 weeks. Experiments were performed at the
end of the 12th week. In vivo erectile responses were measured before and after
intracavernosal injection of onion ... The current data suggest that red onion
juice has a restorative effect on erectile function and endothelium-dependent
relaxation response following the treatment of dutasteride" - See

Novel predictive risk factor
for Erectile Dysfunction: Serum folic acid - Andrologia 2020 Nov 3 -
"The purpose of this study was to compare the serum
Folic Acid (FA) levels in patients with Erectile Dysfunction (ED) and healthy
controls and whether levels vary with its severity. The study was carried out on
77 sexually active individuals, out of which 41 complained of ED and 36 were
apparently normal. Patients were excluded if they had any diseases known to
cause ED ... FA levels were significantly decreased in ED group (5.29 vs. 10.8;
p value = .004). Smoking habits were comparable between the groups, and FA
levels did not vary among smokers and nonsmokers (p value = .46). Serum FA
levels significantly declined with increasing severity of ED (8.28 vs. 5.56 vs.
4.37 vs. 3.5; p value < .001). Thus, decreased FA might possibly be one of the
novel risk factors for ED" - See folic acid products at Amazon.com and

Novel predictive risk factor
of erectile dysfunction: Serum 25-hydroxy vitamin D - Andrologia 2020 Jul 29
- "The present study aimed to investigate the
association between the severity of erectile dysfunction (ED) and serum
25-hydroxy vitamin D. It also sought to determine the cut-off level of serum
25-hydroxy vitamin D for ED. This study included 130 men who had ED between 2018
and 2019. Patients were divided into three groups according to their scores on
the international index of erectile function-5 (IIEF-5) Turkish validated short
form questionnaire. The serum 25-hydroxy vitamin D results were compared between
the groups. The mean age of the patients was 49.28 ± 13.62 years. Groups 1, 2
and 3 included 44 (33.8%) patients with severe ED, 56 (43.1%) patients with
moderate ED and 30 (23.1%) patients with mild ED, respectively. Statistical
significance was observed between the groups and serum 25-hydroxy vitamin D
levels. A positive correlation was detected between the IIEF-5 scores, serum
testosterone and serum 25-hydroxy vitamin D levels. A cut-off level for serum
25-hydroxy vitamin D was calculated as 27.32 ng/ml. During multivariate
analysis, we found that serum 25-hydroxy vitamin D levels were independent
prognostic risk factors for decreased IIEF-5 scores. Decreased serum 25-hydroxy
vitamin D levels were associated with decreased IIEF-5 scores. Therefore,
vitamin D replacement therapy may improve symptoms" - See

Melatonin Prevents
Deterioration of Erectile Function in Streptozotocin-Induced Diabetic Rats via
sirtuin-1 Expression - Andrologia 2020 Jun 1 - "The
objective of this study was to evaluate the protective effects of melatonin for
erectile function with sirtuin-1 protein expression in type 1 diabetic rat
models. Fifty male Sprague Dawley rats were placed into five groups. Except for
those in the control group (C), each animal received a single dose (60 mg/kg) of
streptozotocin to induce diabetes. The animals were placed into the diabetes (D)
group, insulin (I) group (6 U/kg/day), melatonin (Mel) group (10 mg kg-1 day-1 )
and combined treatment (I + Mel) group. Ten weeks later, the serum testosterone
levels, intracavernosal pressure (ICP), mean arterial pressure (MAP),
malondialdehyde (MDA), cyclic guanosine monophosphate (c-GMP),
8-hydroxydeoxyguanosine (8-OHdG), nitric oxide synthase (NOS), caspase-3
activity, sirtuin-1 and endothelial nitric oxide synthase (eNOS) protein
expression and histological findings were assessed. The mean ICP/MAP ratio for
the D group was lower than the mean ratios for the other groups. The treatment
groups, particularly the I + Mel group, exhibited lower 8-OHdG and MDA levels
and caspase-3 activity than the D group. The sirtuin-1 and eNOS expression and
cavernosal tissue (CT) histology seemed to have been preserved by the melatonin
and/or insulin therapy. These results were indicative of a profound protective
effect of melatonin by the activation of sirtuin-1 protein expression against
hyperglycemia-induced oxidative CT injury" - See

Plants in the management
of male infertility - Andrologia. 2020 Jan 28 - "For
many of these medicinal plants such as Lepidium meyenii, Rupus coreanus,
Tribulus terrestres, Panax ginseng, Petasites japonicas, Apium graveolens,
Eurycoma longifólia, Pedalium murex, Corchorus depressus, Mucuna pruriens,
Astragalus membranaceus, Nigella sativa, Crataegus monogyna, Fagara tessmannii,
Phaleria macrocarpa, Anacyclus pyrethrum, Cynomorium songaricum and Morinda
officinalis, the mechanism of actions of their active principles and crude
extracts has been shown in both laboratory animals, in vitro, and human studies,
and includes their antioxidant, anti-inflammatory, spermatogenesis-inducing,
aphrodisiac, smooth muscle relaxing and androgenic properties. Several active
chemical leads including glucosinolates, anthocyanins, protodioscin,
ginsenosides, sesquiterpenes, phyto-oestrogens, quassinoids, diosgenin,
thymoquinone, proanthocyanidins and bajijiasu isolated from these plants are
known to have target effects on the testis, but efforts have been limited in
their application at the clinical level. There still appear to be many more
extracts of medicinal plants that have not been characterised to determine the
phytochemicals unique to them that have target effects on the gonads. Further,
collaborative efforts at isolating pro-drug candidates from medicinal plants for
studies at the molecular, cellular and clinical level towards elucidating their
mechanisms of action on the testes are therefore warranted in the light of the
current male fertility crisis"

Fish Oil Supplements Tied to Sperm Health - NYT, 1/17/20 -
"Researchers studied 1,679 Danish men taking physical
examinations for compulsory military service. Their average age was 19, and 98
of them reported taking fish oil supplements during the previous three months
... Compared with those who took none, men who took fish oil supplements for at
least 60 days during the previous three months had higher sperm count, a greater
proportion of normal sperm cells, higher average semen volume and larger average
testicular size. They also had significantly lower follicle-stimulating hormone
levels and lower luteinizing hormone levels, both indicators of better
testicular function" - See omega-3 supplements at Amazon.com.

Effect of lycopene on
testicular androgenic and anti-oxidative enzymes in cyproterone acetate-induced
male infertile Wistar strain albino rats: An in vitro study - Andrologia.
2020 Jan 8:e13494 - "Incidents of male infertility are
mushrooming worldwide. Oxidative stress plays a prime role for its onset.
Considering this background, the study was designed to focus the direct role of
lycopene on cyproterone acetate (CPA) induced testicular hypofunction in rat.
Four groups have been considered including the vehicle-treated control,
lycopene-treated control, CPA-treated and CPA+ lycopene-treated groups.
Androgenic, antioxidant and toxicity profiles were assessed ... lycopene
exposure to the testicular tissue of CPA-treated rat (CPA+ lycopene-treated)
exhibited a significant (p < .05, p < .001) rectification in testicular catalase,
superoxide dismutase, peroxidase, glutathione-S-transferase activities towards
the vehicle- and lycopene-treated control groups. Toxicity profile also showed a
significant (p < .001) recovery in CPA-treated group after direct exposure of
lycopene towards the vehicle- and lycopene-treated control groups. So, it can be
concluded that direct exposure of lycopene may rectify the CPA-induced
testicular hypofunction either by its free radical-quenching ability or by
stimulating antioxidant enzyme activity without modulating androgenic key enzyme

melatonin and curcumin in the testis of young and aged rats - Andrologia.
2018 Nov 22:e13203 - "Aged Melatonin (MLT; n = 6, 10 mg/kg, s.c.); (d) Aged
Curcumin (CUR; n = 6, 30 mg/kg, i.p.). At the end of 21 days, the rats were
sacrificed, and testicular tissues were removed. Malondialdehyde (MDA) in the
testicular tissue was determined with thiobarbituric acid reactive substances
formation, and glutathione (GSH) was determined with modified Ellman method;
testosterone level was determined with chemiluminescence method and histologic
changes were determined with Haematoxylin-Eosin and Johnsen's scoring; Apoptotic
cell counts were made with TUNEL staining of seminiferous tubule in testis. With
ageing, MDA level increased in testicular tissue, but GSH and blood testosterone
levels decreased. Melatonin treatment for aged rats significantly decreased
Paired total testicular/body weight ratio compared to aged control group (p <
0.05). Curcumin treatment for aged rats significantly increased GSH level
compared to the aged control group (p < 0.05). Besides, melatonin and curcumin
treatment significantly decreased the number of apoptotic cells and
significantly increased Johnsen's score" - See

Resveratrol treatment may
preserve the erectile function after radiotherapy by restoring antioxidant
defence mechanisms, SIRT1 and NOS protein expressions - Int J Impot Res.
2018 Jul 5 - "Radiotherapy (RT) for prostate cancer (PC)
can cause erectile dysfunction (ED) by damaging neurovascular structures with
oxidative stress. In this study, we evaluated the effects of resveratrol, an
antioxidant, on post-RT ED. Fifty rats in five groups were evaluated; control
(C), prostate-confined radiotherapy with short- and long-term vehicle or
resveratrol treatment ... Radiotherapy caused a decrease in sirtuin-1, nNOS, and
eNOS protein expressions. These parameters were reversed by resveratrol
treatment ... Resveratrol preserved the metabolic pathways involved in erectile
function and provided functional protection. Resveratrol can be used as a
supplementary agent in patients undergoing radiotherapy to preserve erectile
function" - See

The effect of low vitamin
D status on sexual functioning and depressive symptoms in apparently healthy
men: a pilot study - Int J Impot Res. 2018 Jul 5 - "Hypovitaminosis
D is associated with sexual dysfunction in apparently healthy women and with
erectile dysfunction in men with the presence of concomitant disorders. This
study was aimed at investigating sexual functioning and depressive symptoms in
young men with low vitamin D status. The study included 15 apparently healthy
men (18-40 years old) with vitamin D deficiency (group A), 16 subjects with
vitamin D insufficiency (group B), and 16 men with normal vitamin D status
(group C) ... Compared with healthy men, subjects with vitamin D deficiency
obtained lower scores for erectile function, orgasmic function, and sexual
desire, while individuals with vitamin D insufficiency lower scores only for
erectile function. Erectile dysfunction was more prominent in group A than group
B. The total BDI-II score was higher in group A than in the other study groups.
The results of the study suggest that low vitamin D status impairs male sexual
functioning and severity of sexual dysfunction correlates with the degree of hypovitaminosis D" - See

Improvement of penile
neurogenic and endothelial relaxant responses by chronic administration of
resveratrol in rabbits exposed to unpredictable chronic mild stress - Int J
Impot Res. 2018 Jan 30 - "Chronic stress is an important public health problem
known as a risk factor for depression, cognitive deficits, and also erectile
dysfunction (ED). Resveratrol, a plant polyphenol, was reported to activate
constitutive endothelial nitric oxide synthase (eNOS) ... Both the electrical
field stimulation (EFS)-induced neurogenic and carbachol-induced
endothelium-dependent relaxant responses significantly decreased in
physiological stress and resveratrol treatment exhibited a marked improvement in
these relaxation responses in vitro. Our results indicated that chronic
psychological stress could lead to ED by reducing neurogenic and
endothelium-dependent relaxations and resveratrol prevents impairment of the
functional responses, suggesting a potential new treatment approach for
treatment of ED during psychological stress" - See

Effect of Kaempferia
parviflora on sexual performance in streptozotocin-induced diabetic male rats
- Andrologia. 2017 Mar 10 - "A significant increase in
sperm density in diabetic rats was observed (p < .05) at highest dose of KP.
Furthermore, KP treatment demonstrated a significant recovery of sexual
behaviour and serum testosterone levels in diabetic rats. These results confirm
that KP exhibits aphrodisiac properties that improve the sperm density,
testosterone level and sexual performance of STZ-induced diabetic rats" -

Effect of rutin on
diabetic-induced erectile dysfunction: Possible involvement of testicular
biomarkers in male rats - Andrologia. 2016 Oct 26 -
"Diabetic-induced alterations in male sexual behaviour and sperm count, motility
and viability were markedly corrected following 5 weeks of rutin treatment to
the diabetic animals. Rutin also attenuated the inhibited serum testosterone and
penile cGMP content, while improved diabetic-associated inflammation and
testicular lipid peroxidation and oxidative stress. Histopathological evaluation
revealed damaged testicular tissues in diabetic rats, which was protected
following rutin treatment. In conclusion, treatment with rutin improved sexual
functionality and also protects against diabetic-induced testicular damage"

Waist circumference is
superior to weight and BMI in predicting sexual symptoms, voiding symptoms and
psychosomatic symptoms in men with hypogonadism and erectile dysfunction -
Andrologia. 2016 Jul 12 - "We investigated whether three
measurements of obesity, weight, body mass index and waist circumference,
correlate with the International Index of Erectile Function-5 (IIEF-5), the
Aging Males' Symptoms (AMS) and the International Prostate Symptom Score (IPSS)
questionnaires ... waist circumference was significantly inversely proportional
to IIEF-5 and directly proportional to AMS and IPSS. Weight was inversely
proportional to IIEF and directly proportional to IPSS but not associated with
AMS. BMI had no proportionality to measurements of sexual function and quality
of life. These results suggest that among weight, BMI and waist circumference,
the latter is the best predictor of health-related QoL in men with hypogonadism"

Korean Ginseng Berry
Extract Effect on Sexual Function in ED - Medscape, 5/6/13 -
"were administered 4 tablets of either standardized
Korean ginseng berry (SKGB, 350 mg ginseng berry extract per tablet), or
placebo, daily, for 8 weeks. Efficacy was assessed with the International Index
of Erectile Function (IIEF)-15 and premature ejaculation diagnostic tool (PEDT)
at the end of the 4th and 8th week. We observed that the total and each of the
individual domain scores of IIEF-15 increased from 40.95+/-7.05 to 46.19+/-12.69
significantly in the SKGB by the 8th week (P<0.05). The erectile function domain
of IIEF changed slightly from 17.17+/-2.57 to 18.59+/-5.99 in the SKGB group by
the 8th week (P<0.05). In addition, PEDT scores significantly improved from
9.14+/-4.57 to 7.97+/-4.4 and 7.53+/-4.26 in the SKGB group after 4 and 8 weeks
of treatment (P<0.05). Safety markers including hormone and lipid in the blood

2013 Jan 17 - "This study aimed at comparing the effects
of atorvastatin and vitamin E on erectile dysfunction in patients initially
irresponsive to sildenafil, with investigation into the underlying possible
mechanisms. Sixty patients were randomly divided into three groups: the
atorvastatin group received 80 mg daily, the vitamin E group received 400 IU
daily and the control group received placebo capsules ... glutathione peroxidase
(GPO) ... nitric oxide (NO) ... Both atorvastatin and vitamin E showed a
statistically significant GPO increase (P<0.05) and a statistically significant
IL-6 decrease (P<0.05). Only atorvastatin showed a statistically significant
increase in NO (15.19%, P<0.05), eNOS (20.58%, P<0.01), IIEF-5 score (53.1%,
P<0.001) and Rigiscan rigidity parameters (P<0.01), in addition to a
statistically significant decrease in CRP (57.9%, P<0.01). However, SOD showed a
statistically significant increase only after vitamin E intake (23.1%, P<0.05).
Both atorvatstain and vitamin E had antioxidant and anti-inflammatory
activities. Although activating eNOS by atorvastatin was the real difference,
and expected to be the main mechanism for NO increase and for improving erectile
dysfunction" - See

The use of
dehydroepiandrosterone in the treatment of hypoactive sexual desire disorder: A
report of gender differences - Eur Neuropsychopharmacol. 2012 Oct 17 -
"Data regarding the efficacy of dehydroepiandrosterone
(DHEA) in the treatment of hypoactive sexual desire disorder (HSDD) are scarce
and inconsistent. We aimed to determine possible gender differences in the
efficacy of DHEA as a treatment for HDSS. Postmenopausal women (n=27), and men
(n=21) with HSDD, were randomized to receive either DHEA 100mg daily or placebo
for 6 weeks in a controlled, double blind study ... In women only, significant
interaction effects were observed for sexual arousal (p<0.05), satisfaction
(p<0.05), and cognition (trend; p=0.06). For arousal, a significant improvement
was observed for the DHEA treated group at 6 weeks (p=0.001). Significant
correlations were observed between bioavailable T and sexual cognitions, arousal
and orgasm, while DHEAS was correlated with satisfaction. In the men,
significant correlations were observed between testosterone and arousal (r=.45),
sexual drive (r=.50) and orgasm (r=.55). In women with HSDD, DHEA treatment had
a significant beneficial effect on arousal, whereas no efficacy was demonstrated
in men, indicating a possible gender difference. This improvement seems to be
mediated via DHEA's metabolism to testosterone. Our positive results suggest
that the neurosteroid DHEA may be effective as a treatment for women with HSDD

Propionyl-L-carnitine, L-arginine and niacin in sexual medicine: a nutraceutical
approach for erectile dysfunction - Andrologia. 2011 Oct 4 -

one-arm study to evaluate the effects of a 3-month supplementation with
propionyl-L-carnitine, L-arginine and niacin on their sexual performance ...
Analyses on GAQs revealed that treatment improved erections in 40% of cases,
with a partial response occurring in up to 77% of enrolled subjects. These
preliminary findings indicate that the favourable cardiovascular effects of
nutraceuticals might also reflect on male sexual function with possible
implication in the treatment and prevention of ED. This study documents a

Effect of
α-lipoic acid on sperm quality, reproductive tract measures in thinner exposed
rats - Andrologia. 2011 Sep 15 - "The aim of this
study was to investigate the effect of alpha lipoic acid (ALA, an universal
antioxidant) on thinner-induced testicular toxicity regarding spermatological
features, body and reproductive tract measures in rats. Adult male
Sprague-Dawley rats were divided into five treatment groups, eight rats in each.
Control group was treated with placebo. Group O was given only olive oil. The
group L received only α-lipoic acid. Thinner + Lipoic Acid group received
thinner + α-lipoic acid and group T received only thinner. Thinner alone
administration caused significant decreases in body and some reproductive organ
weights, sperm count, motility and sperm membrane integrity, and significant
increases in seminal vesicle weight and abnormal sperm rates compared with the
values in the control group. However, concomitant administration of thinner with
α-lipoic acid provided significant improvements in sperm parameters compared
with values in alone group T. In conclusion, the results of this study suggest
that α-lipoic acid has a protective effect against thinner-induced reproductive
dysfunction in male rats"

Assessment of a Supplement of Pycnogenol® and l-arginine in Japanese Patients
with Mild to Moderate Erectile Dysfunction - Phytother Res. 2011 May 27 -
"Subjects were instructed to take a supplement
(Pycnogenol® 60 mg/day, l-arginine 690 mg/day and aspartic acid 552 mg/day) or
an identical placebo for 8 weeks, and the results were assessed using the
five-item erectile domain (IIEF-5) of the International Index of Erectile
Function. Additionally, blood biochemistry, urinalysis and salivary testosterone
were measured. Eight weeks of supplement intake improved the total score of the
IIEF-5. In particular, a marked improvement was observed in 'hardness of
erection' and 'satisfaction with sexual intercourse'. A decrease in blood
pressure, aspartate transaminase and γ-glutamyl transpeptidase (γ-GTP), and a
slight increase in salivary testosterone were observed in the supplement group.

males aged between 25 and 52, without erectile dysfunction and randomized to an
oral dose (two tablets per day) of the active treatment (600 mg Testofen per
day) or placebo for 6 weeks. The primary outcome measure was the DISF-SR (male)
self-administered QOL total score and the four domain scores. The secondary
outcome was specific quality of life parameters. Testofen had an overall
positive effect on physiological aspects of libido. In particular, there was a
significant increase in the subdomains of sexual arousal and orgasm. Testofen
had a positive effect on QOL in self-reported satisfaction with muscle strength,
energy and well-being but did not have an effect on mood or sleep. Serum
prolactin and testosterone levels remained within the reference range. It was
concluded that Testofen demonstrated a significant positive effect on
physiological aspects of libido and may assist to maintain normal healthy
testosterone levels" - See

Oral
L-citrulline supplementation improves erection hardness in men with mild
erectile dysfunction - Urology. 2011 Jan;77(1):119-22 -
"L-arginine supplementation improves nitric
oxide-mediated vasodilation and endothelial function; however, oral
administration has been hampered by extensive presystemic metabolism. In
contrast, L-citrulline escapes presystemic metabolism and is converted to
L-arginine, thus setting the rationale for oral L-citrulline supplementation as
a donor for the L-arginine/nitric oxide pathway of penile erection ... A total
of 24 patients, mean age 56.5 +/- 9.8 years, were entered and concluded the
study without adverse events. The improvement in the erection hardness score
from 3 (mild ED) to 4 (normal erectile function) occurred in 2 (8.3%) of the 24
men when taking placebo and 12 (50%) of the 24 men when taking L-citrulline (P <
.01). The mean number of intercourses per month increased from 1.37 +/- 0.93 at
baseline to 1.53 +/- 1.00 at the end of the placebo phase (P = .57) and 2.3 +/-
1.37 at the end of the treatment phase (P < .01). All patients reporting an
erection hardness score improvement from 3 to 4 reported being very satisfied"

A new
herbal combination, Etana, for enhancing erectile function: an efficacy and
safety study in animals - Int J Impot Res. 2009 Jun 4 -
"a new herbal combination called Etana that is
composed of five herbal extracts including Panax quinquelotius (Ginseng),
Eurycoma longifolia (Tongkat Ali), Epimedium grandiflorum (Horny goat weed),
Centella asiatica (Gotu Kola) and flower pollen extracts. Most of the
above-mentioned extracts have a long historical and traditional use for
erectile dysfunction (ED) ... penile erection index (PEI) ... At a 7.5 mg
kg(-1) single dose, the percentage of responding rats was 53+/-7 with a PEI
of 337+/-72 compared with 17+/-6 with a PEI of 30+/-10 for control animals.
This PEI was significantly (P<0.001) higher than each single component and
than the sum of any two herbal components of Etana. When compared with
sildenafil citrate, Etana induced more pronounced PEI than 0.36 mg kg(-1),
but similar to 0.71 mg kg(-1) of sildenafil. Furthermore, full acute and
sub-acute toxicity studies showed no toxic effects of Etana. In conclusion,
this study describes a new and safe combination of herbal components that
enhance erectile function in male rats"

Antioxidant treatment
with alpha-tocopherol improves erectile function in hypertensive rats -
Hypertens Res. 2008 May;31(5):1007-13 - "Treatment
with 24 mg alpha-tocopherol decreased TBARS levels and increased SOD
activity in the serum and corpus cavernosum. Relaxation in response to
acetylcholine chloride in the corpus cavernosum was improved with
alpha-tocopherol treatment at each dose. These results suggest that
alpha-tocopherol treatment increases the diminished relaxation in the corpus
cavernosum of SHRs by improving neuronal or endothelial function related to
nitric oxide and carbon monoxide. This, in turn, indicates that antioxidant
therapy may play a role in treatment for ED in hypertensive patients"

Can we rely

erectile dysfunction? - Int J Impot Res 2022 Mar 28 -
"Although male hypogonadism (MH) is a
prevalent comorbidity in patients presenting for erectile
dysfunction (ED), its screening relies solely on total
testosterone (TT). Ageing and other conditions can increase sex
hormone-binding globulin (SHBG) and lower free testosterone (FT)
causing symptomatic MH despite normal TT. The primary objective
was to measure the prevalence of normal TT/low FT among patients
presenting for ED ... Normal TT/low cFT is frequent and can be
missed by current screening recommendations for MH in patients
presenting for ED. Ageing seems to be the main culprit as
elevated SHBG prevalence increases steeply after the sixth
decade. TT cannot solely be relied on to exclude biochemical MH
in patients presenting for ED, especially in patients over 60
years old"

Letrozole once a week
normalizes serum testosterone in obesity-related male hypogonadism - Eur
J Endocrinol. 2008 May;158(5):741-7 -
"Isolated hypogonadotropic hypogonadism (IHH) is
frequently observed in severely obese men, probably as a result of increased
estradiol (E(2)) production and E(2)-mediated negative feedback on pituitary
LH secretion. Aromatase inhibitors can reverse this process ... treated with
2.5 mg letrozole once a week for 6 months ... Six weeks of treatment reduced
total E(2) from 123+/-11 to 58+/-7 pmol/l (P<0.001, mean+/-s.e.m.), and
increased serum LH from 4.4+/-0.6 to 11.1+/-1.5 U/l (P<0.001). Total
testosterone rose from 5.9+/-0.5 to 19.6+/-1.4 nmol/l (P<0.001), and free
testosterone from 163+/-13 to 604+/-50 pmol/l (P<0.001). Total testosterone
rose to within the normal range in all subjects, whereas free testosterone
rose to supraphysiological levels in 7 out of 12 men ... Letrozole 2.5 mg
once a week produced a sustained normalization of serum total testosterone
in obese men with IHH. However, free testosterone frequently rose to
supraphysiological levels. Therefore, a starting dose <2.5 mg once a week is
recommended"

Losartan Improves
Erectile Dysfunction in Diabetic Patients - Medscape, 12/3/12 -
"In this study, we found that diabetic patients
suffering from ED were responsive to the losartan treatment. The IIEF-5 scores,
the percentage of positive answers to the SEP-2, SEP-3 and GAQ after treatment
are all significantly increased as compared with baseline and the end point of
control groups. The analysis of our study also indicated that losartan worked
better in mild and moderate ED than severe cases. It is consistent with our
previous studies, which shows that losartan can downregulate the local
expression of AT1 and Ang II, but it cannot elevate the reduced smooth
muscle/collagen ratio in diabetic rats [7,8] We believe that the losartan may
not completely reverse the severe damaged penile tissues due to diabetes"
- Note: It says that's the first study in diabetics but I've read several
studies over the years supporting losartan and ED. See my ARB page. It would
seem that it would extend to the entire ARB class of antihypertensives but I
haven't seen studies on that. See losartan (Cozaar)

Association
of metabolic syndrome, atherosclerosis risk factors, sex hormones in ED in
aboriginal Taiwanese - Int J Impot Res. 2012 Apr 12 -
"International Index of Erectile Function (IIEF)
questionnaire ... Using age-adjusted multivariate logistic regressive analysis,
this study showed that aboriginal males with ED had a significantly higher
prevalence of MS (OR=12.02, 95% confidence intervals (CI): 6.33-22.83, P<0.001).
Among the MS components, abnormal fasting blood sugar was the most significantly
independent factor for ED in aboriginal males (OR=8.94, 95% CI: 4.71-16.97,
P<0.001). The presence of MS had a significant correlation with lower IIEF-5
scores, lower sexual desire scores, lower testosterone serum level (P<0.01) and
abnormal interleukin-6 (IL-6) and high sensitivity C-reactive protein (HsCRP).
The results of this study support the idea that MS, low serum testosterone and
HsCRP may predict ED in aboriginal Taiwanese males" - See

Serum
testosterone levels in diabetic men with and without erectile dysfunction -
Andrologia. 2012 Apr 4 - "TT was lower in diabetic men
with ED as compared to those with normal erectile function (EF), 392.4 +/- 314.9
versus 524.3 +/- 140.2 ng dl(-1) , respectively ... Receiver operating
characteristic (ROC) curve of TT in men without associated co-morbidities showed
that EF was compromised at TT = 403.5 ng dl(-1) or less. Sensitivity of 63.3%
and a specificity of 94.0% were detected. At this level, ED was found in 33/38
(86.8%) men with TT 403.5 ng dl(-1) , whereas ED was observed in 57/147 (38.8%)
men with TT ≥ 403.5 ng dl(-1) (P < 0.0001). We propose a cut-off value of 403.5
ng dl(-1) of TT blood levels as an indicator for initiation of testosterone

The
Relationships between Sex Hormones and Sexual Function in Middle-Aged and Older
European Men - J Clin Endocrinol Metab. 2011 Aug 17 -
"The aim of the study was to investigate the
relationships between serum testosterone (T), estradiol (E2), and
dihydrotestosterone (DHT) and sexual function in a multicenter population-based
study of aging in men ... Total and free T, but not E2 or DHT, was associated
with overall sexual function in middle-aged and older men. E2 was the only
hormone associated with sexual function-related distress such that higher levels
were related to greater distress. Free T levels were associated with
masturbation frequency and erectile dysfunction in the fully adjusted models,
such that higher T was associated with less dysfunction and greater frequency.
Moreover, there was a T threshold for the relationship between total T, sexual
function, and erectile dysfunction. At T concentrations of 8 nmol/liter or less,
T was associated with worse sexual functioning, whereas at T levels over 8
nmol/liter, the relationship came to a plateau. Conclusions: These findings
suggest that different hormonal mechanisms may regulate sexual functioning (T)
vs. the psychological aspects (E2) of male sexual behavior. Moreover, there was
a T threshold for overall sexual function such that at levels greater than 8
nmol/liter the relationship between T and sexual function did not become
stronger"

Weight
Loss Improves Erections in Obese Men With Diabetes - Medscape, 8/10/11 -
"For the study, researchers recruited obese men who had
body mass indexes (BMIs), a number calculated from weight and height, of greater
than 30 and waist sizes over 40 inches ... The average age of men in the study
was 60 ... men were randomly assigned to either a low-calorie diet, where two
daily meals were replaced by weight loss shakes, or to a higher-protein,
lower-carbohydrate eating plan ... In the healthy eating plan, which focused on
three meals of lean protein, five daily servings of vegetables, and two daily
servings of whole grain carbohydrates and fruit, was designed to shave about 600
daily calories off what the men would normally eat ... After eight weeks, the
men on the low-calorie meal replacement diets were switched to the healthy
eating plan, and all continued to be followed for a year ... After eight weeks
on the diets, the men on the low-calorie plan had lost about twice as much
weight as the men on the healthy eating plan, 20 pounds vs. 11 pounds ... But
they had similar improvements in erectile function, sexual desire, and urinary
symptoms ... Of the 17 men with severe erectile dysfunction, Wittert says that
after eight weeks, seven had improvements in their symptoms, with some achieving
normal function. After a year, 25% had normal erectile function"

function from 4.8+/-3.1 to 3.4+/-2.9; sexual desire from 3.4+/-2.9 to 4.4+/-0.6;
overall sexual satisfaction from 2.6+/-1.3 to 3.8+/-1.0). But scores of the IIEF
did not normalize. Frequency of weekly sexual intercourse increased from
0.4+/-0.4 to 1.1+/-0.8 (P<0.05) after 6 months. There were no adverse effects"

hypogonadism - Eur J Endocrinol. 2008 May;158(5):741-7 -
"Isolated hypogonadotropic hypogonadism
(IHH) is frequently observed in severely obese men, probably as a
result of increased estradiol (E(2)) production and E(2)-mediated
negative feedback on pituitary LH secretion. Aromatase inhibitors
can reverse this process ... treated with 2.5 mg letrozole once a
week for 6 months ... Six weeks of treatment reduced total E(2) from
123+/-11 to 58+/-7 pmol/l (P<0.001, mean+/-s.e.m.), and increased
serum LH from 4.4+/-0.6 to 11.1+/-1.5 U/l (P<0.001). Total
testosterone rose from 5.9+/-0.5 to 19.6+/-1.4 nmol/l (P<0.001), and
free testosterone from 163+/-13 to 604+/-50 pmol/l (P<0.001). Total
testosterone rose to within the normal range in all subjects,
whereas free testosterone rose to supraphysiological levels in 7 out

lead to treatment noncompliance. Nitric oxide synthase (NOS) is an important
cellular modulator of erectile function. We have therefore investigated the
effect of antipsychotic drug on activity and gene expression of NOS in rat
penile tissues. The activity of constitutive NOS was significantly
suppressed below control by a 21days administration of 1mg/kg haloperidol,
which also significantly decreased expression of endothelial NOS (eNOS) and
neural NOS mRNA. Risperidone at 0.5mg/kg also reduced eNOS mRNA expression.
