Blood Test Order Form

The way the Life Extension Foundation Blood Tests works is that you order the blood test.  They mail you the blood test request forms, signed by their doctor, along with a prepaid UPS envelope.  You take the forms to a local facility.  I use Healthsouth Medical Clinics, which is a popular chain in San Diego.  They charged me $15 to draw the blood and mail it.  About a week later, I received the results in the mail.
